_____ Date from the Graduate Office when my next RC is due: _________

1^st RC is held no later than one calendar year after passing Prelim, and at least annually thereafter unless otherwise stated by Committee.

_____  Read Advisory Committee Expectations

_____  2 weeks prior to your RC date, notify the Graduate Office with the:

1.  Location
2. Date
3. Time

_____  1week prior to your RC provide your committee with a completed PHD-RC student question.pdf form.

_____  2-7 business days before your RC the other two forms will be sent to you via email from the Graduate Office.

1.  Advisory Committee Report of Research Conference
2. Prospective Questions for Master Students - Committee

_____ Day of meeting, give the above paperwork (3 documents in total) to your committee chair.

_____  Return all paperwork to the Graduate Office, no later than the 2-days after your RC
